  • Caregiver Support Services
    There are two service programs available: National Family Caregiver Support Program and Kentucky Family Caregiver Program.

  • Hart-Supported Living
    This program provides funds for supports to help Kentuckians with disabilities live in their homes and communities, avoid institutionalization, and to comply with Olmstead requirements.

  • Long-Term Care Ombudsman
    This program advocates for residents of nursing homes, personal care homes and family care homes, improving care and encouraging positive change at all levels.

  • Participant Directed Services
    This program allows people eligible for Medicaid waiver services to choose their own providers for nonmedical waiver services.

  • State Health Insurance Assistance Program (SHIP)
    This free program provides information, counseling and assistance to seniors and disabled individuals, their families, and caregivers. It helps the public and Medicare beneficiaries make informed decisions about health care. SHIP does not sell health insurance products.

  • Traumatic Brain Injury Trust Fund
    This program provides flexible funding and support for community-based efforts to meet the special needs of persons with a brain injury.

  • Nutrition Program for the Elderly
    This program provides one nutritionally balanced home-delivered or congregate dining-style meal per day. Additional services include nutrition screening, education, and counseling for persons aged 60 or older and their spouses, regardless of age.
